At Pinnacle Charter Schools, weโre excited to offer a tuition-free, accredited education for students in grades 6-12. Our dedicated team is here to guide you through a streamlined registration and admission process, ensuring a smooth transition into our vibrant learning community.
Registration Process
To enroll your child at Pinnacle Charter Schools, please complete the following steps:
- Complete the Registration Form: Submit this form to your chosen school or district office, or fill out our Online Application for convenience.
- Provide Proof of Age/Identity and Arizona Residency: Ensure you submit the required documents as detailed below:
Proof of Age/Identity (required within 30 days of registration):
- A certified copy of the studentโs birth certificate.
- Alternative documents such as a baptismal certificate, an application for a social security number, or original school registration records, accompanied by an affidavit explaining the absence of a birth certificate.
- A letter from an authorized representative of an agency with custody of the student as per juvenile court proceedings.
Note: Proof of age and identity is not required for homeless students as per A.R.S. ยง 15-828(l).
Proof of Arizona Residency
- Complete the Arizona Department of Education Residency Documentation Form or the State of Arizona Affidavit of Shared Residence.
- Submit a copy of one of the verifiable documents listed on these forms.
Note: Proof of residency is not required for homeless students as per 42 U.S.C. ยง 11 432(g)(3)(C)(i).
The studentโs or their guardiansโ race, color, national origin, immigration status, or other impermissible factors are not considered in establishing residency.
Admission Process
Supplemental Documents: While not mandatory for admission, providing these documents helps us better serve your student and tailor their Personalized Learning Plan. We strongly encourage you to submit:
- McKinney-Vento Homeless Education Assistance Survey.
- Proof of Immunization, a Medical Exemption Form, or a Personal Beliefs Exemption Form if the student attends a Site School. By state law (A.R.S. ยง15-873), a child cannot attend school until proof of immunization or a completed exemption form is submitted. (Homeless students have a five-day grace period to submit proof or exemption forms.)
- A Medical Exemption Form, or Personal Beliefs Exemption Form (A.R.S. ยง15-873). Homeless students have a five-day grace period to submit proof or exemptions.
- Primary Home Language Other Than English (PHLOTE) Form.
- Official or unofficial high school transcripts.
- Official or unofficial middle school semester report cards and progress reports.
- Withdrawal Form: From the previous school.
- Middle School Promotion Certificate: Or other verification from the previous school.
- Special Education Records: If applicable.
- Proof of Custody or Guardianship: Or a letter from an authorized representative of an agency with custody of the student.
- Standardized Test Scores.
- English Language Learner testing scores (AZELLA/SELP).
- Notice to Provider form.
- Primary Caregiver form.
